| Patient Forms: When you
enter the office at Lakeside Sports Chiropractic Center, you will
be greeted by our professional staff and welcomed as a new patient
into the Practice. We will ask that you fill out our new patient
forms, which can be found in the New Patients Information. This
will include your general information as well as a thorough health
history questionnaire. Click

Consultation: On your initial visit, you will have the opportunity
to sit and speak at length with Dr. Jennifer Lidstrom to carefully
review and discuss your health history, concerns and potential treatment
options.
Examination & Treatment: After your consultation, Dr. Lidstrom
will conduct a complete exam, consisting of thorough orthopedic,
neurologic, chiropractic, radiologic (if necessary), and soft-tissue
examinations. This will allow her to make a determination on your
course of treatment. If at any time during your exam or treatment
there is a need for other tests such as blood work, MRI, or consultation
with a specialist, we will provide you with a referral to one of
the qualified facilities or providers whom we work with regularly.
We encourage a team approach while you are reaching your wellness
goals, through open communication with you, as well as your primary
care physician.
